INTERVENTION:
i.
Provide a description of the frequent and information about maintenance
procedures and how to decrease fear and anxiety clarify misconceptions
and improve co-operation.
ii. Indicate the desire to hear and speak to the patient when the procedure is pain free
iii. Access metal status including mood, affective.
iv. Encourage patient to talk about injuries everyday
v.
Explain to the patient what is happening. Give a chance to ask question
and give answers to open/honest and show the reality of the situation
of compensation that can help the patient.
Diagnosis V: The risk
infection associated with decreased system, failure to resolve
infection, traumatic tissue injury i.e. patient evaluation.
INTERVENTION:
i. Access for signs of infection as early ditesi infection
ii.
Perform Nursing aseptically and antiseptic to prevent cross
contaminations and prevent exposure to infections germs or organism.
iii. Remind the patient not to hold the wound and soaking the injured area to prevent contamination of wounds
iv. Teach washing hands before and after contact with client to prevent cross contamination reduce the risk of infection
v. Check the wound daily, watch and record changes in appearance odor of the wound
3.6 SCIENTIFIC PRINCIPLES OF INTERVENTION
Scientific
principles are the application of interventions used in order to
promote and enhance positive attitude of individuals and family
concerned.
Scientifically, I succeeded in convincing Mrs. “Nâ€
family to have the knowledge and importance of protection against snake
bite by giving her the necessary education about the use of gadgets like
rain boots, hand gloves and rain coats while working in the farm or
rearing animals. I was able to provide her with rain boot, hand gloves
for protection against snake bite of insect stings like scorpion and so
on….
However, the entire communities were very happy and were
able to comply with the health education about environmental sanitation
and personal hygiene especially hand washing and toilet facilities. It
also motivated the entire community to sending their children to the
same university.
EDUCATION
The effects of intervention of Mrs. “N†case during the subsequent follow up visit are as follows:
1.
I notice that the entire compound was busy which attracts snakes and
other reptiles or living organisms that are harmful to man.
2. I noticed also that there was no toilet facility; they use their nearby bush as their toilet facilities.
3.
Even though households were moderately cool, but there was no enough
space to accommodate the number of people living in each room
4. The internal compound was very neat and well swept.
5. I noticed a drastic change in the activities of the entire community as they cleared the surrounding bushes.
6. During another visit, I was told that Mrs. “N†almost had an attack by another snake as she was in the bush for toilet.
3.6 SUBSEQUENT HOME VISITS
Home
visit is a professional visit to a client by the health worker in order
to achieve a look and close assessment of their mental, physical,
spiritual and social status within the context of their own environment.
Such visit can be scheduled or unscheduled when the client is inform
about the visit and (unscheduled) without informing the client (Adeyemi
T) so as to enable me carry out my study or research work successfully
