      All information was obtained from the patient case file and history from the patient and relative
      3.3    PATIENT FAMILY AND SOCIAL HISTORY
      Alhaja A.H, happily married in a polygamous setting with five children.  She does not smoke nor drink alcohol but take local herbs when she feel she is sick.  Prior to the illness, patient was happy but now anxious about the prognosis of the disease
      3.4    PAST MEDICAL HISTORY
      Patient has once been admitted in General Hospital Ilorin for a case of malaria fever. patient has no history of surgery or any chronic illness before her current situation of health
      3.5    PRESENT HISTORY OF ILLNESS
      Patient was apparently well until she notice a boil on the right foot which was treated with a local ointment and later developed into an ulcer.  She was brought to accident and emergency unit on 30th march 2017.
      On examination, patient was conscious, alart and pulse wooing out from the ulcer.  She was diagnosed having diabetes right foot ulcer

          Respiration             22c/m
          Blood pressure         140/90mmHg
