      MANAGEMENT OF THE PATIENT WITH DIABETES MELLITUS
      4.0    PHYSICIAN SYSTEMIC REVIEW
          The findings made on the patient based on systemic review are as follows
          PHYSICAL EXAMINATION
      Physical examination of the extremity having a diabetic ulcer can be divided into 3broad categories
      ï‚§    Examination of the ulcer and the general condition of the extremity
      ï‚§    Assessment of the possibility of vascular insufficiency
      ï‚§    Assessment for the possibility of peripheral neuropathy
          CARDIO VASCULAR SYSTEM
      ï‚§    Blood pressure     150/90mmHg
      ï‚§    Pulse rate         88beat per minute
      ï‚§    Apex beat         74beats per minute
      ï‚§    Heart sound         No murmur
      RESPIRATORY SYSTEM
      Respiratory rate is 22 cycles per minutes.  The chest expansion is clinically clear.  There is no cough and chest pain
      CENTRAL NERVOUS SYSTEM
          The patient was conscious, alert and well oriented to time, place and person.  She also responded to question appropriately
      GASTRO – INTESTINAL TRACK
          There was no history of abdominal pain, abdominal distention, loss of bladder or bowl tone, vomiting with loss of appetite
      MUSCULO SKELETAL SYSTEM
      There is tinging sensation and weakness in the lower limbs
      REPRODUCTIVE SYSTEM
      Pruritus of the vulva develops due to infection by fungi which thrive better on glucose deposit from the urine.  The vulva becomes swollen and inflamed as a result of the frequent itching or irritation.


      4.3    PHYSICIAN DIAGNOSIS
      The diagnosis made by the doctor based on systemic review and result of laboratory investigation, the patient was discovered to be suffering from DIABETES MELLITUS RIGHT FOOT ULCER
      4.4    MEDICAL MANAGEMENT OF THE PATIENT
      The drug prescribe for the patient are as follows
      Tab     Nifedipine 20mg bd daily
      Tab     Glucophage 500mg b.d
      Tab   Captopril 25mg twice daily
      Subcutaneous soluble insulin 20iu 8hourly
      Tab     vitamin B complex
