    [A CASE STUDY OF A PATIENT WITH DIABETES RIGHT FOOT ULCER]

    • 4.7    HEALTH EDUCATION
          According to primary health care lecture note (Kwara state college of nursing and midwifery Ilorin).  Health education is a behavioural changed communication that is aimed towards improving the health standard of individual, group or family.  In other word, health education is the health talk given to individual, group of people or community in order to present disease, promote quick recovery and to prevent recurrence or relapse of illness.
          Alhaja A.H and her relative were health educated on the following
          She was adviced to avoid white sugar
          She was also encourage to comply with drug regiment
          She was advice to regularly carry out checkup to prevent re-occurance
          Adequate good personal and environmental hygiene where also emphasized to prevent infection
          She was advice to join diabetes diabetes association for more enlightment on the disease
      4.8    REHABILITATION
          It is a way of adopting her to her new way of life.  To bring patient back to her former state of health, physically, mentally and socially or atleast bring her closer to what she was before the illness the rehabilitation of the patient begins from the day she was admitted until time she was discharge.  She was assisted on physical care like oral hygiene, assisted bathroom bath and changing of bed linen.
          The relatives were also involved in her care by telling them what to do and not to do such as the type of food they should prepare for the patient
      4.9    DISCHARGE
      Patient was discharged home, when her condition has improved and the blood sugar 10.2mmol/L.  she was discharged with some prescribed drug and appointment date was given for follow up.
      4.10    HOME VISIT
      Home visitation is to promote
          Promote Nurse –patient relation
          To assess likely environmental contributory factor to her health condition
          To bring scientific medical care to the patient home
      The patient leave  in a cluster building surrounding with stagnate water and dirty cloth litered around, the wound site is granulating rapidly and the patient was encouraged on good balanced diet and adequate personal hygiene
          The patient complain of severe body pain and inability to carried out daily activities, it was discovered that her blood pressure was high and advice to see the doctor.  She was anxious about her state of health.
